Polish Community Aged Care Packages
(CACP)



Polish Community Aged Care Packages make it possible for older people to enjoy quality of life whilst living in their own homes. The "packages of care" are individually tailored to suit the needs of each client and may involve a combination of:

  • Personal care

  • Emotional and social support assistance with meals and dietary requirements.

  • Domestic assistance

  • Transport for shopping and appointments

  • Other assistance as negotiated individually

Who is eligible?

  • Elderly people who have complex needs: medical/physical/social and /or emotional needs requiring a package of care.
     
  • People who have been assessed by an Aged Care Assessment Team as requiring Low Level Care.

The Federation of Polish Organisations provides care packages to the Polish community in the metropolitan area of Adelaide. The Polish Community Aged Care Packages Program is funded by the Commonwealth Department of Health and Ageing.

 

Services provided are regularly monitored and can change to suit different circumstances. If a major change in the client’s or carer’s health occurs and the programme is unable to meet their needs, then options for alternative care will be discussed with the client and their advocate. Assistance will be given to find alternative arrangements

 

A service Fee is charged, this fee is negotiable and dependant on the client’s financial situation.

 

We can arrange mobility aids for clients with special needs:

  • Wheelchairs

  • Shower/toilet chairs

  • Walking frames

  • Rails on the bathroom/toilet for those requiring assistance

   

After a person becomes eligible for the Package, the coordinator will arrange a home visit with the person and his/her advocate to discuss their needs.
A package of services will then be negotiated and a service agreement completed.
